General Registration Information Registration is open to all surgeons, audiologists, speech pathologists, and professional partners in the field of pediatric cochlear implantation. Registration fees for all categories include: All scientific sessions, panels, and poster displays Sponsor and Industry Support Displays All continental breakfasts and refreshment breaks daily Buffet lunch daily Friday evening reception Registration Fees On or Before May 22, 2009 Physicians - $575 Nonphysicians - $475 Students - $200 After May 22, 2009 Physicians - $675 Nonphysicians - $575 Students - $200 Purchase orders will not be accepted. Payment must be paid in full at the time of registration. Deadline for Registration Early registration must be postmarked by Friday, May 22, 2009. Registrations received and postmarked after May 22, 2009, will be charged according to the pricing structure published on the registration form. Student Registrants A copy of your student identification card must be attached to the registration form. The identification card must be current and will also be required at the Cochlear Registration Desk to pick up your registration materials. Confirmation Formal confirmation will be sent to all attendees within 10 business days of receipt. All badges/credentials will be distributed on site at the conference registration desk. Cancellation Requests for refunds must be made in writing and received on or before May 22, 2009. There is a $50 handling fee for all refunds and returned checks. Cancellations and registrations postmarked after the deadline date will not be eligible for refunds.
